Management. Ultimately, they're more concerned with themselves than the growth of their employees.
No trust. We're adults, with lives outside of work. Sometimes you need to go to the dentist, need to run home to pick up a package, etc. Leaving work is often considered blasphemy. Clock watching runs rampant here.
Bro sales culture is rampant. If you don't drink the coolaid, you'll be an outsider. Different learning styles and personalities are often overlooked if you're not suckling at the company teet.

Pay is pretty bad - especially considering what other companies in the area are paying.
Work/Life balance is almost nonexistent. People are consistently expected to work over the weekends, 12 hour days, etc. All for the "greater good" of the company.
