Out of touch with their remote teams - we were working for a customer every day during the pandemic, but nobody knew that. Virtually no contact with their remote teams, no performance feedback for years. We wouldn't have even received raises to cover inflation if our prime didn't fight for us. When I resigned they wouldn't let me take my paid vacation time and stole it from me. I should have take the vacation time, then quit and left the customer with minimal notice.
