Some managers and HR leaders are living in a parallel reality, they don't trust nor believe their employees and lower managers when they ask for help, because it's easier for them to ignore or downplay problems instead of taking on eventual conflicts. Typical example is avoiding to hire additional employees in teams that are overloaded and telling the employees that their overload is their fault because they are not efficient enough.
Some lower managers are really not grown up to their roles. They just try to keep every employee, even non-performers who just make other team members do their part of the job.
