Good interview with several key players in the company. From executive, HW, direct report, to peer level. Not very forthcoming with culture, benefits, and work life. Good technical discussion with examples testing technical proficiency.

You are contacted via email by someone who represents himself as the Hiring Manager. If still interested, he emails you again with several interview questions and based on your responses, sets up a phone interview.
The initial conversation with this individual is quite social and you are thinking that based on your prior email responses that this interview was more aimed at getting a read on your personality and ability to effectively communicate vs more questions.
However this was not to be the case. He then asks 4 or 5 additional questions most of which were hypothetical leaving out critical details that if reversed, it's highly doubtful that even he could answer.
And now for the coup de grâce! At the end of the interview, he informs you that he is actually not the hiring manager and that he is writing down all of your answers which are to be submitted to a hiring committee which will determine your fate.

Something like, "How would you handle a supplier that was consistently late on deliveries?"
