I applied online. The process took 12 months. I interviewed at NIH in Mar 2011
Interview
Very informal. 5-6 questions about my previous experience. Very specific. We talked by phone. I was in my home country. They were in the US. My English was then not the best. But they understood that. I could re-ask some things and several times they did not understand my pronunciation. But still everything was good.
