Permanent Medical Officer

Sub-Acute Care & Rehabilitation Centre | Somerset West

Faircape Health Institute is extending our subacute care and rehabilitation services to Heritage Manor in Somerset West, providing restorative care for patients requiring short-term, long-term, dementia, respite, palliative, and rehabilitation-focused care within a secure and nurturing environment.

Working within a highly collaborative multidisciplinary environment, our focus is on medical stability, rehabilitation progression, chronic disease management, and supporting patients on their journey from hospital to home.

We are seeking a clinically confident Permanent Doctor/Medical Officer to join our growing multidisciplinary healthcare team at Heritage Manor. This role is best suited to a doctor comfortable managing medically complex rehabilitation and long-term care patients within a ward-based environment.

The successful candidate will work closely with our multidisciplinary team, prioritising patient stability, rehabilitation progression, chronic disease management, and continuity of care within a structured healthcare environment.

Duties include but are not limited to:

  • Conduct comprehensive medical assessments on admission, including physical examinations, medical history reviews, and interpretation of diagnostic results to inform care planning.
  • Develop and implement individualised treatment plans, including medication management and rehabilitative interventions aimed at improving outcomes.
  • Take ownership of ongoing clinical management and medical decision-making for medically stable rehabilitation and long-term care patients.
  • Manage patients with multiple co-morbidities, functional decline, chronic disease burden, and rehabilitation-related complications.
  • Collaborate with the multidisciplinary healthcare team to ensure continuity and coordination of patient care.
  • Provide clinical guidance and support to nursing teams within the facility.
  • Monitor patient progress, evaluate treatment effectiveness, and adjust care plans as required.
  • Communicate clearly with patients and families, providing education on medical conditions, treatment plans, and self-care strategies.
  • Maintain accurate and up-to-date medical records on Healthware (electronic platform), including progress notes and discharge summaries.
  • Participate in quality improvement initiatives, including protocol development and implementation of best practices in subacute care.
  • Provide urgent/emergency telephonic medical support to nursing staff when required to stabilise patients and prevent deterioration.
  • No routine call-outs from home are required.
  • Establish and maintain professional relationships with referring specialists relevant to the Somerset West catchment area.

Requirements:

Qualifications

  • Bachelor of Medicine & Bachelor of Surgery (MBChB)

Experience and Knowledge

  • 3 years + post-community service experience
  • Experience in internal medicine, family medicine, geriatrics, or palliative care is beneficial.
  • Experience working within a multidisciplinary and nursing team environment advantageous.

Specific Requirements

  • Clear health, criminal, and credit record
  • Written references
  • MPS/insurance membership and active HPCSA registration
Working Hours:
  • Monday to Friday: 08h00 – 17h00
  • Approximately 2 weekends per month required operationally
  • Weekend rounds generally conclude around 12h00 / 13h00
  • Urgent/emergency telephonic support to nursing staff when required
  • No routine call-outs from home required
