Licensed Practical Nurse (LPN/LVN) — Job Description

Overview Provide safe, compassionate basic nursing care under RN or physician supervision to support patient recovery, comfort, and daily needs across clinical settings.

Key Responsibilities

• Perform nursing assessments: take vital signs, record patient history changes, and report abnormalities to RN/physician.
• Administer medications and injections per scope and policy; monitor for adverse reactions and document accordingly.
• Provide wound care, dressing changes, catheter care, IV site observations, and basic nursing procedures within scope.
• Assist with activities of daily living (ADLs): bathing, dressing, toileting, feeding, mobility assistance, and transfers.
• Monitor and document patient status, input/output, pain levels, and response to treatments; update care plans as directed.
• Collect specimens (urine, stool) and perform basic point-of-care testing (blood glucose, dipstick) as allowed.
• Collaborate with RNs, physicians, therapists, and interdisciplinary team for care coordination and discharge planning.
• Educate patients and families on medications, wound care, and home-care instructions within LPN scope.
• Maintain accurate, timely documentation in patient records and follow charting protocols.
• Participate in infection control, safety protocols, and medication reconciliation processes.
• Supervise and delegate tasks to nursing assistants or CNA staff as permitted by facility policy; precept students when applicable.
• Participate in quality improvement, in-service training, and maintain licensure requirements.

Qualifications

• Graduate of an accredited practical/vocational nursing program.
• Current LPN/LVN license in the practicing state/province/region.
• BLS/CPR certification required; ACLS/PALS may be required for certain settings.
• Experience: entry-level to 3+ years depending on role; experience in long-term care, clinics, or acute settings as preferred.
• Strong clinical skills in medication administration, basic wound care, and patient assessment.
• Good communication, organization, and teamwork skills; ability to prioritize tasks in fast-paced environments.
• Physical ability to assist with patient transfers and perform duties requiring standing and lifting.

Working Conditions

• Settings: long-term care, skilled nursing facilities, hospitals (medical-surgical), clinics, home health, and outpatient centers.
• Shift work including nights, weekends, and holidays; possible on-call requirements.
• Exposure to biological hazards and need for PPE; physically demanding tasks.

Performance Metrics

• Medication administration accuracy and documentation completeness.
• Patient safety incident rates and infection-control adherence.
• Timeliness of vital-sign monitoring and reporting.
• Patient and family satisfaction scores.
• Compliance with licensure, training, and competency requirements.

Pay: $5,750.35 – $6,250.45 per month
