Question 1
What does this section of the billing form represent?
I applied online. The process took 3 weeks. I interviewed at Premiere Medical Resources in Apr 2009
Interview
I applied for a specific IT position. I was left a voice mail inviting me to schedule an interview with them. When I responded, they told me that they had already filled the position. I thought this to be very unprofessional.
However, they did follow up with me, to see if I would be interested in another IT position and invited me to an interview. I put the first experience down as an honest mistake, and decided to give them a chance. The interview went well. It comprised of meeting three of their managers.
I was invited for a second round interview to meet with the Business Owner who was the Chief Physician along with one of the manager's from the first interview. I felt that the interview got off to a bad start. The physician game me the impression that I needed them much more than they needed me. He leaned back in his chair, crossed his arms, and asked, "So why do you want to work here?" in a tone that seemed elitist. It got better after that after he warmed up to me. I also met with their technical staff. I wasn’t offered the job which was a good thing. I think the environment would have been a bit authoritarian.

Question 1
They said at one point something like, and 'I assume you've had experience with [blank]'. It apparently was important, and they seemed surprised. I never claimed I did in my cover letter or my resume.
